Why are Navy Warships being rammed by Commercial boats?

You would think with or without sophisticated navigation equipment a Navy Destroyer could avoid an oil tanker. BTW, the latest casualty is the USS John McCain - I assume named after the Senators father. Is this Trump payback for the healthcare vote? (Just kidding.)
Two accidents in three months is unbelievable.
