I believe its the NCAA, not the B1G.
From what I've read, it's the B1G that grants the medical redshirt. If the redshirt is denied, then the athlete can appeal to the NCAA who may then grant the redshirt.
NCAA grants 6th years. Conferences grant medical redshirts.
