Japanese hitter just stepped on Home Plate multiple times w/ his swing step...

....that is illegal, no? The umpire did not call it despite it happening multiple times. Pretty sure you are not allowed to step on Home Plate while swinging - am I wrong about that and just remember the rules incorrectly?

Didn't see the play, but you gotta hit the ball while out of the box to be called out. If he's swinging and missing, it's just a strike.

The penalty is specified in 6.02(c) "If the batter refuses to take his position in the batter’s box during his time at bat, the umpire shall call a strike on the batter. The ball is dead, and no runners may advance. After the penalty, the batter may take his proper position and the regular ball and strike count shall continue. If the batter does not take his proper position before three strikes have been called, the batter shall be declared out."

But if you line-up out of the batters box, especially repeatedly, it is supposed to be called a strike even if you don't swing, no? Several times he lined up on top of plate outside of box, stepped further forward (onto plate several times) without swinging, but ump did not call the strike. Believe batter can be warned, but if the batter does it repeatedly (e.g., lines up outside of Batters Box) a strike is supposed to be called whether he swings or not, no? Pretty sure it is, especially if you repeatedly step on plate before ball has reached home plate.
 
Its Little league, I highly doubt they follow MLB rules.

Pretty sure virtually every league begins with MLB Rulebook and makes modifications downward from there. Do not believe this is a rule that is modified (the distance from Pitchers mound and down the base-paths is one of the rules that is modified for instance...).

Also noticed in several games the catcher frequently sets up outside of the catchers box, which is illegal, and on intentional walks leave the box before the pitcher released the ball. Obviously they are not so strict on enforcing some rules.

Again these are kids playing, not even teenagers yet. I do think some of the rules are different. I mean MLB players don't even understand the rules you guys brought up.

That rule is probably for High School baseball. Every league has there own rules and adoptions. Little League definitely does not follow MLB rules. I am not sure what the rule is for Little League, but I know the leagues I umpire for you have to be making contact with the ball while touching the plate or stepping out of the batters box to be called out.

The rule (6.02(c)) is for between pitches. The batter has to keep one foot in the box when he is looking down at the coach for signals. You are supposed to give a warning, then call a strike. In 14 years I have never called this one, but I have called a batter out for stepping out of the box a few times.
