Assuming he fully recovers, does he immediately apply for a 6th year, and play as a RS freshman next year? How does that work?

You apply for a "6th" after you use up your "standard eligibility".......IIRC

I do not believe there is even a process to do that ahead of time
 
So next year, then, he's a RS sophomore. Ok, I just didn't know how it worked.

You can apply for a 6th year ahead of time.

Garry Gilliam applied during his senior season.

Wasn't aware that he sat out 2015 due to an injury. If not, then he's not eligible for a sixth year. Either way, it's too early.
 
As Art implied above, I believe a player can only apply successfully for ta 6th year if they have been injured for at least 2 full years. (full years as defined by the NCAA's rule on redshirting - can't have played in more than 3 games???)
